Omprazole

Omprazole( Benzimidazole).

  • Esomprazole is the S-isomer of omeprazole.
  • A specific equine product has been developed, because oral bioavailability of the human omeprazole formulation or compounded formulations is poor in horses.
  • IV injectable human formulations of omprazole can be considered for use in animals.

MOA:

  • Parietal cell screte H+ions into gastric lumen . The enzyme H+/K+-ATPase plays role in this secretion .
  • Omprazole is proton pump inhibitor and irreversibly block the H+/K+-ATPase proton pump of the gastric parietal cell.
  • The inhibitory effect of single dose omprazole remains for 3-4 days in dog and horse.

Effects:

  • Decrease in acid secretion.
  • Healing of gastric ulcers.

Side effects:

  • Long term use of omprazole has following side effects:
  • Hyperplasia and hypertrophy of the gastric rugae.
  • Development of carcinoids.
  • Renal failure
  • Disorders of calcium homeostasis, including fractures.

Species :

  • Cattle, Horse , Pig ,Dog and Cat.

Indication:

  • Hyperacidity.
  • Gastritis.
  • Gastric ulceration.
  • Abomasal ulceration.
  • Helicobacter Pylori infection.

Drug combinations:

  • Combined with Amoxicillin/Tetracycline, Metronidazole and Bismuth subsalicylate for Helicobacter Pylori infection.

Dose:

Gastric ulcers:

  • 0.5-1mg/kg and 0.5-1.5mg/kg PO OD in Cat and Dog respectively.
  • 4mg/kg PO OD X 4 weeks in Horse.
  • 2mg/Kg PO OD as preventive dose in horses.
  • 2mg/kg IV and 4mg/kg PO in ruminants and milk fed calves respectively.
  • 40mg total dose in Swine.

Acute gastritis:

  • 0.7mg/kg PO OD in Cat and Dog.

Helicobacter Pylori infection in Dogs:

  • 0.66mg/kg PO with Amoxicillin /Tetracycline, Metronidazole and Bismuth subsalicylate for 2-3weeks.

Contraindication:

  • prolong use.
  • Use cautiously in liver and Kidney diseases.

Brands:

  • Kick-ulcer 6.15g oral paste syringe ( Veko India).

Composition: Each 6.15g syringe contains 2.28 gm omprazole.

Cimetidine

Cimetidine( Antihistamine).

  • Extra label drug.

MOA:

  • Cimetidine inhibits H2 receptors in stomach thus decreasing acid secretion.

Effects:

  • Antacid.

Side effects:

  • Rare in animals.

Species :

  • Dog and Horse.

Dose:

  • 5-10mg/kg PO OD in dog.
  • 2.5mg/kg PO Q8hr or 5mg/kg OD X 3 months in horse.

Indication:

  • GIT ulcer in dog.
  • Megaoesophagus in dog.
  • Acid reflux in dog.
  • Equine Multiple Melanomas.

Contraindication:

  • Pets allergic to other H2 Blockers.
  • Liver failure.
  • Renal failure.

Cisapride

Cisapride (Benzamide group)

  • Prokinetic drug .

MOA:

  • Cisapride stimulates 5-HT4 receptors in the intestine.
  • There is release of acetylcholine from pre synaptic sites.
  • The net effect is increased propulsive movement of intestine and secretory reflex.

Effects :

  • Increase in peristalsis.

Side Effects:

  • GIT upset like vomiting and diarrhea.

Serious side effects:

  • Nervous sign like incoordination, excessive drooling, seizures .

Species:

Dog and Cat.

Indication:

  • Idiopathic constipation.
  • Megacolon in cat.
  • Post operative ileus.

Dose:

  • 0.1mg/kg PO TD in dog.
  • 2.5mg/cat PO TD in cat weighing less than 5kg .
  • 5mg/ cat same route in cat weighing more than 5kg.
  • Give after eating for best results.

Contraindication:

  • Mechanical obstruction.
  • Impaction.

Maropitant

Maropitant( Antiemetic)

  • NK-1 antagonist.
  • Approved for treatment of general emesis in dogs and cats and is considered a safe and effective drug.

MOA:

  • Substance P in the vomiting center of the brain binds to neurokinin-1 receptors, causing vomiting. Substance P also modulates pain perception by signaling the intensity of noxious stimuli.
  • Along with NK1, substance P is likely involved in the body’s response to stress, anxiety, invasion of territory, and noxious or aversive stimuli.
  • Maropitant binds as antagonist with neurokinin (NK1) receptors and blocks the action of Substance P.

Effects:

  • Antiemesis.
  • Anti nociception.
  • Anti inflammatory.

Side effects:

  • Slight pain or irritation at injection site. The pain can be reduced if refrigerated injection is used( At cold temperature formulation remains intact).
  • Excess salivation and muscle tremors.

Species:

  • Dog.

Indication:

  • Prevention and treatment of general emesis in the dog.
  • Prevention and treatment of chemotherapy induced emesis in the dog.
  • Prevention of motion sickness in the dog.
  • Nausea related to travel in car.

Dose:

  • 1 mg/kg SC or 2 mg/kg PO OD X5 days.

Contraindication:

  • Puppies younger than 11 weeks old ( causes bone marrow hypoplasia).

Brands:

  • Cerenia 16mg,24mg, 60mg and 160 mg tablets (Pfizer USA-Offered by GNH India).
  • Cerenia 20 ml injection vial(Pfizer USA-Offered by GNH India).

Composition: Each ml contains 10 mg of maropitant as maropitant citrate


Metoclopramide

Metoclopramide (benzamide derivative).

  • Extra label drug for dog and cat.
  • No effect on normal GIT motility.

MOA:

  • Anti emetic activity: Metoclopramide act as antagonist at D2 receptors in the chemoreceptor trigger zone in the central nervous system — this action prevents nausea and vomiting triggered by most stimuli. At higher doses, 5-HT3 antagonist activity may also contribute to the anti emetic effect.
  • Gastroprokinetic activity: This action is mediated by muscarinic activity, D2 receptor antagonist activity, and 5-HT4 receptor agonist activity. The gastroprokinetic effect itself may also contribute to the anti emetic effect.
  • Metoclopramide also increases the tone of the lower esophageal sphincter.
  • Metoclopramide might influence on mood because of its blockade action on 5-HT4 and 5-HT3.

Effects:

  • Anti emesis and nausea relief.
  • Increase in Gut movement.

Side-effects:

  • Dizziness.
  • Sedation.
  • Confusion.
  • Anxiety.
  • Hyperactivity.
  • Behavioral changes.
  • Fluid retention.
  • Increased urination.
  • Increased seizures in epileptic dogs.
  • Constipation.
  • Diarrhea.
  • Abdominal pain.
  • Nausea.

Indication:

  • Nausea and vomiting associated with chemotherapy and as an anti emetic for dogs with parvovirus enteritis.
  • Treatment of acid reflux.
  • Treatment of abomasal emptying defects in sheep and vagal indigestion in cattle.

Drug combinations:

  • Metoclopramide (0.1 mg/kg SC)+ Bethanechol (0.07 mg/kg SC) for Increase in abomasal and duodenal motility in cattle.
  • Metoclopramide+ Simethicone for gastrointestinal evacuation.
  • Metoclopramide+ Dimethicone gastrointestinal evacuation.

Dose:

  • 0.25mg – 0.5 mg/Kg PO/ IM/SC/IV TID/QID as antiemetic for dog and cat.
  • 1mg -2mg/Kg PO/ IM/SC OD as prokinetic for dog and cat.
  • Should be given 30 minutes before meal.
  • 0.1mg- 0.5mg/Kg IM/SC as prokinetics for ruminants.

Contraindication:

  • GIT obstruction.
  • GIT bleeding and perforation.
  • Pheochromocytoma.
  • Allergy to sunscreen containing PABA.
  • Seizures.

Mirtazipine

Mirtazipine( Tricyclic antidepressant).

  • Appetite stimulant.
  • Approved as transdermal application in cats.

MOA:

  • Mirtazapine inhibits presynaptic α2-adrenergic action on autoreceptors and heteroreceptors in the central nervous system. This leads to enhanced noradrenergic and serotonergic activity.
  • Mirtazapine is a strong antagonist of serotonin 5-HT2(5-hydroxytryptamine 2) and 5-HT3 receptors. It also indirectly increases 5-HT1A transmission.
  • Mirtazapine is also a peripheral α1-adrenergic antagonist. This may lead to orthostatic hypotension.
  • Mirtazapine is a potent histamine (H1) receptor antagonist, thus may have sedating effects.

Effect:

  • Reduced anxiety and increased sedation.
  • Boost appetite and gain weight.
  • Anti Vomiting effect.
  • Anti nausea effect.

Side-effects:

  • Drowsiness.
  • Hyperactivity was reported in 11% of cats using the transdermal gel.
  • More affectionate behavior is reported.
  • Increased vocalization.
  • Serotonin syndrome e.g tremors/shivering, dilated pupils, difficulty breathing, elevated body temperature, high heart rate or high blood pressure and hypersensitivity.

Dose:

  • 0.6mg/kg PO OD in dog.
  • 3 – 4mg total dose PO every 72 hours in cat.
  • 1.5-inch ribbon of transdermal ointment (approximately 2 mg/cat equal to 0.1 mL) on the inner pinna of the cat’s ear once daily for 14 days.

  • Cat and Dog.

Indication:

  • Nausea and vomiting.
  • Appetite loss.
  • Behavioral problems.

Contraindication:

  • Simultaneous use of MOA inhibitors such as Amitraz and seligiline.
  • Simultaneous use of SSRI such as fluoxetine.
  • Liver disease and blood disease.
  • Hypersensitivity to drug.

Mitratapide and Dirlotapide

Mitratapide and Dirlotapide (chemically related to the antifungal drugs such as itraconazole).

  • Antiobese drugs.
  • Approved for weight loss in dogs.

MOA:

  • Mitratapide inhibits microsomal triglyceride transfer protein (MTP) at enterocyte.
  • The microsomal triglyceride transfer protein is responsible for processing of triglycerides in enterocytes so there is accumulation of triglycerides inside the enterocytes.
  • The accumulation of these triglycerides in the intestinal cells sends a signal to the CNS to suppress the appetite.

Effects:

  • Decrease in postprandial serum triglycerides, phospholipids, and cholesterol.
  • Decrease uptake of triglycerides by enterocyte.
  • Decrease in appetite.
  • Weight loss.

Side effects:

  • Vomiting.
  • Diarrhea.
  • Anorexia.

Species:

  • Dog.

Indication:

  • Management of obesity in dogs.

Dose:

  • Mitratapide: 0.63 mg /kg OD PO X 8weeks.
  • Dirlotapide: 0.05 mg/kg OD PO X 13 days, then 0.1mg/kg on day 14 .
    The dose is then adjusted based on the weight loss goals for your pet. The drug is continued for the period of 1 year.

Contraindication:

  • Previous sensitivity.
  • Liver disease.
  • Dogs taking corticosteroids.

Magnesium hydroxide

Magnesium hydroxide ( Milk of Magnesia).

  • Antacid and luxative.
  • Used in rumen acidosis.

MOA:

  • Magnesium hydroxide in rumen reacts with lactic acid to form magnesium lactate and water.
  • In true stomach it reacts with HCl to form magnesium chloride and water.
  • In intestine it causes release cholecystokinin (CKK) by which water and electrolytes accumulate in the lumen.
  • In colon it draws water into lumen and increase water content in lumen.

Effects:

  • Neutralizes effect of lactic acid in rumen.
  • Neutralizes effect of hydrochloric acid in stomach.
  • Laxative effect in large intestine.

Side effects:

  • Decreases number of protozoa in rumen.
  • Substantially increases rumen pH.
  • Reduces fermentation.
  • Diarrhea and depletion of potassium.

Therefore, these compounds should only be administered to cattle with a confirmed diagnosis of grain overload.

  • Carbohydrate engorgement.
  • Hyper acidity.
  • Constipation.

Dose:

100–300g PO BD/TD in cattle.

10–30 g PO BD/TD in sheep.

Contraindication:

  • Dehydration and renal failure.

Chlorpromazine

Chlorpromazine (phenothiazine derivative).

  • Centrally acting antiemetic.

MOA:

  • Chlorpromazine blocks histamine H1, dopamine D2, and muscarinic M1 receptors in the vomiting center.

Effects:

  • Antiemetic effect.
  • Sedation.

Side effects:

  • Tiredness.
  • Low blood pressure.
  • Tremors, shivering and rigidity
  • Diarrhea and loss of anal sphincter tone.
  • Loss of certain reflexes.

Species:

  • Dogs and Cats.

Indication:

  • Motion sickness.
  • As tranquilizer.
  • Irritable colon syndrome.
  • Muscle relaxation in tetanus.

Dose:

  • Tranquilization: 0.8mg-3mg/kg in dog and 0.25 – 0.5mg/kg in cat.
  • Antiemetic: 3.3 mg/kg in dog .
  • Irritable colon Syndrome: 0.5 mg/kg in dog .
  • Muscle Relaxation during tetanus: 2 mg/kg in dog .

Contraindication:

  • Use cautiously in animals with seizures.
  • Avoid in horses.

Ropinirole

  • Used for treatment of Parkinson’s disease in humans.
  • Noval drug approved for inducing vomiting in dogs.
  • Easily administered.
  • Rapid and short-lived action.
  • Ocular absorption is fast and reliable, bypassing the gastrointestinal tract.

MOA:

  • Ropinirole binds with D2-type receptors in CTZ of brain and acts as agonist.

Effects:

  • Stimulation of CTZ causes vomiting.

Side effects:

  • Hyperemia of eye.
  • Excessive tearing.
  • Third eyelid protrusion.
  • Tachycardia.
  • Antidote: Metoclopramide.

Species:

  • Dog.

Dose:

  • 3.75 mg/meter square, intraocular single dose.

Indication:

  • To make dogs vomit after ingesting toxic or harmful substances.

Contraindication:

  • Seizures.
  • Ocular pathology especially corneal ulceration.
  • Dogs ingested corrosive chemicals and foreign objects.

Brands:

  • Clevor 0.3 mL, prefilled single dose dropper (Vetoquinol USA).

Composition: Each ml contains 30mg Ropinirole.

Administration protocol:

  • Up to 5kgbw 1drop in one eye.
  • Above 5kgbw upto10kgbw 1drop in each eye.
  • Above 10kgbw upto 20kgbw 2drops in one eye and 1 drop in another eye.
  • Above 20kg bw upto 35kgbw 2drops in each eye.

Bisacodyl

Bisacodyl(synthetic pyridinylmethylene-diacetate ester derivative).

  • Stimulant laxative.

MOA:

  • Bisacodyl stimulates neurons in colon and increases peristalsis of colon.
  • It also acts as contact laxative in colon ,increases secretion of salt and fluid.

Effects:

  • Bisacodyl has parasympathetic effect directly on mucosal sensory nerves, increasing peristaltic contractions thus Increases bowel movement.
  • Laxative effect.
  • Relieves constipation.

Side effects:

  • Nausea.
  • Cramps.
  • Diarrhea.

Species:

  • Dog and Cat.

Indication:

  • Constipation.
  • Bowl cleaning before surgery.
  • Proctoscopy and colonoscopy.

Dose:

  • 5mg PO in cat and small dog.
  • 10mg PO in medium size dog.
  • 15-20mg in in large dog.

  • Bowel obstruction.

Hyoscine butylbromide

Hyoscine butyl bromide / Scopolamine (anticholinergic drug).

  • Approved by FDA for equine use.

MOA:

  • Hyoscine binds to muscarinic receptors and blocks the action of acetylcholine, leading to the relaxation of smooth muscles in the GI tract.

Effects:

  • The overall result is to alleviate intestinal spasms, bronchial constriction, and esophageal spasms.
  • Colic pain relief begins within 5–10 minutes and lasts for 3-4 hours.
  • Rectal relaxation makes rectal palpation easier.

Side effects:

  • Transient tachycardia is caused by its parasympatholytic effects.

Drug combination:

  • Hyoscine butylbromide can be used with NSAIDs(metamizole )and sedatives ( Xylazine) to potentiate the antispasmodic and analgesic effects.

Species:

  • Horse and food animals.

Dose:

  • 0.2 mg/kg IV in horse.
  • 0.3mg/kg IV/ IM in cattle.
  • 0.4 mg/kg IM BD X 3days in calf.
  • 0.4 mg/kg IM in pig.

Indication:

  • Treatment of uncomplicated, spasmodic colic in horse.
  • Recurrent airway obstruction in horse.
  • For relieving spasms and pain in cattle and pigs.

Contraindication:

  • Impaction colic associated with ileus.
  • Equine glaucoma.
  • Unstable cardiovascular status.
  • Pre existing tachycardia and homeostatic disturbances like dehydration and volume contraction, electrolyte and/or acid-base abnormalities.
  • Obstruction colic in horse.

Brands:

  • Ketaspas 10ml Injection vial (Medicure SoluItions India).

Composition: Each ml contains 20mg Hyoscine.

  • Buscopan 50 mL Injection vial (Boehringer Ingelheim Vetmedica Germany).

Dipyrone

Dipyrone(Pyrazolone group).

  • Also called Metamizole.
  • Synonyms: Analgin, Novalgin, Methampyrone etc.
  • NSAID approved for horses.

MOA:

Peripheral action:

  • Dipyrone blocks the synthesis of pyrogens.
  • Dipyrone inhibits cyclo oxigenase there by reduces synthesis, of prostaglandin D &E, endoperoxides, bradykinins and free radicals.

CNS action:

  • Dipyrone stimulate the synthesis of beta endorphin that causes the analgesic action on visceral pain.
  • Dipyrone inhibits painful extra-and proprioceptive impulses through Gaulle`s fascicles and Burdach’s columns.
  • Dipyrone raises the threshold for sensitivity to pain in the thalamic centers.

Effects:

  • Peripheral and central actions result in strong pain relief and reduce fever.

Species:

  • Horse.

Indication:

  • Pyrexia in horse.
  • Using Scopolamine and Xylazine combination for horse colic.

Dose:

  • 30mg/kg IV OD/BD X 3days in horse.

Contraindication:

  • Allergy to drug.
  • Concurrent use of other NSAIDS , steroids and nephrotoxic drugs.
  • Use cautiously in horses prone to bleeding.

Flunixin

Flunixin (NSAID)

  • Approved drug for horse and cattle.
  • Rapidly absorbed into the bloodstream, with pain relief and fever reduction typically starting within one to two hours.

MOA:

Dose:

  • 1.1mg/kg IV in equine colic.
  • 0.25 mg/kg with antibiotics for treating sepsis in horses.

  • 1.1-2.2mg/kg IM/IV in camel sheep and goat

Contra indication:

  • Avoid use with other anti-inflammatory drugs such as corticosteroids or other NSAIDs.

  • Extra label drug.
  • First generation sedative antihistamine.

MOA:

  • Cyproheptadine works by blocking H1receptors and 5-HT2 receptors.

Effects:

  • Antihistaminic action.
  • Anti serotonin action .

Side effects:

  • Hyperexcitability in cats.
  • Dry mouth.
  • Tachycardia.

Species:

  • Dog , Cat and Horse.

Dose:

  • 0.3-2mg/kg OD / BD PO for pruritus and appetite .
  • 1.1mg/kg Q 4-6hrs PO until serotonin symptoms disappear.
  • 0.25mg/kg OD/BD for pituitary disease and photic head shaker.

Indication:

  • Pruritus and serotonin syndrome in dog.

Capromorelin (potent ghrelin receptor agonist)

  • Appetite stimulating agent.
  • Approved by FDA for increase in body weight in dogs and cats.

MOA:

  • Capromorelin binds ghrelin receptors, and acts like ghrelin.
  • Ghrelin is a hormone produced by the stomach. It signals the brain via ghrelin receptors when the stomach is empty, making the animal feel hungry and start eating.

Effects:

  • Food consumption and body weight is increased.

Side effects:

  • Vomiting, hyper salivation and lethargy.

Species:

  • Dogs and Cats.

Indication:

  • Anorexia.
  • Weight loss.

Dose:

  • 3 mg/kg PO×4 days.

Contraindication:

Hydrogen peroxide( H2O2).

  • Liquid form readily available over the counter.
  • Cheap and economical.
  • Can be used both as disinfectant and emetic agent.
  • 90% success rate as emetic in dogs.
  • should not be used as emetic agent in cats .
  • More fruitful if used at home immediately after ingestion of poison by pet.

MOA:

  • Hydrogen peroxide irritates the nerves in the throat and stomach, leading to the initiation of the vomiting reflex.

Effects :

  • Vomiting.

Side effects:

  • Gastric irritation.
  • Gastric hemorrhage in case of overdose or when concentration of hydrogen peroxide in solution is more than 3%.
  • Use proton pump blockers or H2 blocker and sucralfate to control the adverse side effects.

Species:

  • Dog.

Indication:

  • Inducing vomiting for poisoning.

Dose:

  • Using dosing syringe adminster hydrogen perxide @2.2 mL/kg (3% solution) PO, the maximum dose is 45 mL /dog.
  • Wait for 10 to 15 minutes, adminster second dose if no emesis is seen.
  • The hydrogen peroxide can be mixed with a few drops of milk or ice cream to encourage voluntary ingestion.
  • Light exercise may increase the chance of emesis.

Contraindication:

  • Cats , as hydrogen peroxide causes fatal hemorrhagic gastritis in them.
  • In case sharp solid object is ingested.
  • When dog has become camatose or shocked or showing moderate to severe signs of poisionig.
